Haglund's exostosis

Haglund's exostosis is a special type of calcaneal lesion with concomitant reactive changes in adjacent tissues. For the first time, the so-called "shoe tumor" heel syndrome was described in 1928 by the Swedish surgeon Haglund, after whom it was named.

Clinically, Haglund's exostosis manifests itself in tendinitis, bursitis and tenodynia at the site of attachment of the Achilles tendon. The external symptoms are characterized by the presence of seals in the area of the calcaneus, pain when pressed and inflammation of the skin.

The causes of Haglund's disease are most often the wearing of shoes that cause discomfort in the heel area. Pumps, so beloved by women, are especially insidious in this regard: the back of such shoes is usually curved in such a way that it digs into the back of the calcaneus, resulting in a spur on the heel, which reduces the distance between the heel and the metatarsal heads. Exostosis can also form on the back of the heel, and inflammation of the skin occurs due to constant mechanical friction against the surface of the shoe. Ski boots and skates are also common culprits in the development of Haglund's disease, so ski enthusiasts should be especially careful.

Haglund's exostosis is diagnosed based on a medical examination and radiography.: The image will show a significant overgrowth of bone tissue at the upper posterior projection of the calcaneus. The main difficulty is that both types of the disease - achillobursitis (between the Achilles tendon and the bone) and bursitis of the subcutaneous calcaneal sac (between the tendon and the skin) - are not easy to distinguish from a simple inflammation of the Achilles tendon.

In the vast majority of cases, conservative treatment of Haglund's exostosis is indicated. A number of physiotherapy procedures are performed to improve blood circulation, analgesics are prescribed for severe pain, and wearing special shoes with orthopedic backs is mandatory. In some cases, the doctor applies a plaster splint to ensure the rest of the limb.

Surgical treatment is extremely rare (2% of all cases): surgical removal of bone and cartilaginous growths. The operation is performed under local anesthesia, the procedure is low-traumatic and does not require long rehabilitation.
